COVID-19 School Exposure Form

The school exposure form is for school administrators, nurses, and school points of contact to submit known school exposures.This form is not for parents or guardians to submit when their child has been exposed in an instructional school setting.

COVID-19 Community Transmission Levels for Schools

Central District Health adopted the Centers for Disease Control and Prevention’s (CDC) COVID-19 Community Levels as a guide for schools. Your community level is measured by county and can be found here. CDC updates the community level every Thursday.

Levels of Community Transmission information helps schools and school administrators stay informed about COVID-19 activity in their communities so they can make decisions around the health and safety of students and staff. Additional information about how the community level is calculated, precautions to take at each level and more can be found here.
