School Physicals

Central District Health Department offers school/sports physicals students in grades 7 thru 12. The cost of a school physical is not available on a sliding fee scale.

Services provided during the school/sports physicals include:

• Physical Exam
• Vision Screening
• Health Promotion Guidance

Immunizations may be recommended. To schedule an appointment, call the Immunization Department at the clinic closest to you.
